چکیده

Purpose. The validity of preoperative MRI in diagnosing HAGL lesions is debated. Various investigations have produced mixed results with regard to the utility of MRI. The purpose of this investigation is to apply a novel method of diagnosing HAGL/pHAGL lesions by looking at contrast extravasation and to evaluate the reliability of such extravasation of contrast into an extra-articular space as a sign of HAGL/pHAGL lesion. Methods. We utilized specific criteria to define contrast extravasation. We evaluated these criteria in 12 patients with arthroscopically proven HAGL/pHAGL lesion. We also evaluated these criteria in a control group. Results. Contrast extravasation occurred in over 83% of arthroscopically positive cases. Contrast extravasation as a diagnostic criterion in the evaluation of HAGL/pHAGL lesions demonstrated a high interobserver degree of agreement. Conclusions. In conclusion, extra-articular contrast extravasation may serve as a valid and reliable sign of HAGL and pHAGL lesions, provided stringent criteria are maintained to assure that the contrast lies in an extra-articular location. In cases where extravasation is not present, the "J" sign, though nonspecific, may be the only evidence of subtle HAGL and pHAGL lesions. Level of Evidence. Level IV, Retrospective Case-Control series.

Combined posterior osseous Bankart lesion and posterior humeral avulsion of the glenohumeral ligaments: a case report and pathoanatomic subtyping of "floating" posterior inferior glenohumeral ligament lesions.

T he pathoanatomy of anterior shoulder dislocations has been well documented. Perthes described an anterior labral lesion as early as 1906, and Bankart wrote his classic article on recurrent anterior dislocations more than seventy years ago. Until recently, much less had been written about posterior shoulder instability; to our knowledge, the first case series was reported by Reeves in 1963. Re...

Combined HAGL and Bankart Lesions in an Adolescent Athlete with Ligamentous Laxity and Recurrent Shoulder Dislocation

This is a report of a 17-year-old wrestler who had suffered from a sport injury during an international competition that led to recurrent shoulder dislocation. Physical examination showed that he had a severe ligamentous laxity. In the magnetic resonance images there were combined humeral avulsion of the glenohumeral ligament (HAGL) and Bankart lesions. Combination of Bankart and HAGL lesions, ...

Evaluation of Diagnostic Visual Findings at Laparoscopy in Endometriosis

Background Endometriosis has been widely implicated as one of the causes of chronic pelvic pain, dysmenorrhea and infertility. The purpose of our study was to define the presumed correlation between visual and histologic diagnostic features of endometriosis based on the peritoneal findings identified in laparoscopy as a diagnostic trial with a standardized technique. So, all the specimens inclu...

Comparative Evaluation of Cell Block Method and Smear Cytology in Fine Needle Aspiration Cytology of Intra-abdominal Mass Lesions

Background& Objective: Fine needle aspiration cytology (FNAC) is an easy, rapid, and less hazardous tool to diagnose the intra-abdominal lesions with various imaging modalities adding to its sensitivity and accuracy. However, sometimes it does not yield adequate information for precise diagnosis and the risk of false-negative and indeterminate diagnosi...

Evaluation of Diagnostic Agreement between Teledermatology and Clinical Examination of Skin Diseases

Background: Telemedicine provides a tele-communication between physician and patient in remote areas for transferring patient information for the purpose of diagnostic and therapeutic services. Teledermatology is a kind of approach for dermatologists that uses high resolution images for diagnosis. In this study we evaluated the agreement between direct (patient-physician) and indirect (telemedi...
